Please provide the names of all guests as they appear on travel documents (passport or driver's license). Exact names are required by law and incur change fees if not provided accurately. If you have passports, please send a picture of the ID page to Suzanne via email for reference.

By checking *yes* I acknowledge that the names and birthdates submitted are EXACTLY as they appear on passports or official travel identification and that any errors or costs incurred as a result of errors are my (the traveler's) responsibility.

Passport offices are advising at least a 6-8 week turnaround time for passports. If you do NOT have a passport it is imperative that you apply immediately. A passport that does not arrive in time for travel is NOT a valid insurance claim.

Travel Insurance. 

Travel insurance provides financial protection and assistance for unexpected events such as trip cancellations, medical emergencies, lost baggage, and other unforeseen circumstances while traveling. Please consider purchasing the recommended insurance. Not doing so leaves you unprotected before and during travel. 

Permissibility to travel.

U.S. citizens traveling to any destination outside of the United States will be required to present a valid U.S. passport. Passports must be valid for 6 months past the return date. Some countries require a visa for transit or entry. It is the passenger’s responsibility to ensure that they have all the proper documents for entry and meet all entry requirements. All names on documents must match the legal name on your photo I.D., and travel document information must match tickets. Please check the State Department website at https://travel.state.gov for further information. Immunizations may also be required. Failure to comply with these regulations may result in denied boarding, denied entry, and/or government imposed fines and are the fault of the passenger. If you are a citizen of another country, there may be additional requirements. Check with the nearest consulate or embassy of the destination you are traveling to and find out the entry requirements for non-U.S. citizens.
